Migraine with aura
Migraine is said to occur as a result of primary neuronal dysfunction. Cortical spreading depression (a wave of depolarization of glia and neurons that makes its way across the cortex and is self-propagating) is associated with migraine aura and headache. This wave of depolarization activates trigeminal afferent neurons. The trigeminal afferent neurons innervate blood vessels in cerebral parenchyma, pia, dura mater, and venous sinuses, thus activating the trigeminovascular system. Once the trigeminal ganglion is activated, vasoactive peptides like substance P and calcitonin gene-related peptides are released, leading to inflammation. Calcitonin gene-related peptide is a potent vasodilator. Hence, as a part of the inflammatory process, vasodilation occurs. Inflammation then leads to a sensitization of neurons, which become increasingly sensitive to nociceptive stimuli.
